You just have to be open and uprfront, tell the doctor that you do not sleep well at night and have trouble falling asleep and that you’d like to try a sleep aid. That’s all I did and my doctor gave me a script for ambien cr.

Serving as a doctor is a regarded walk of life
I agree with DrLove, sometimes being candid and upfront with your doctor works best. Just tell him what you’ve told us. Many healthcare professionals are very receptive to legitimate concerns that people have, even if it is embarrassing, awkward, or whatnot. Give it a try!
Good luck!
