I know what derealization is, although the only time I experienced it (profoundly) was once on LSD 30 years ago.
My husband takes Apidexin. He is 6′ tall and was 211 lbs. That was three weeks ago. He is now about 192. the stuff does its job, although I have tried it and I do NOT feel it works as well for me. I lost 5 lbs in same time. Keep in mind, I’m menopausal, on high blood pressure meds, and wellbutrin.
I felt it did make me more anxious, and on two occassions, had to take an ATIVAN to come myself. So, is it worth it?
Not to me…5 lbs is really not a big deal.

I think you would have to be supervised VERY closely, and if that’s the case, you might as well ask for the big guns, like MERIDIA which will help you lose weight much more effectively.
Good luck.
